An encounter with the strange creature called The Maxx, who has made friends with Horridus, sends both he and The Maxx for a bizarre trip through the Dragon's subconscious, or, his "outback." They are retreived once Maxx's companion, Sarah, interrupts them. While the villainous Brainiape tries to rob a bank without the consent of the Vicious Circle, he is stopped by their agent, Powerhouse. Dragon and Rapture both suspect each other of infidelity.

In Hell, the Fiend sends Spawn to Earth, and sics Overlord on the Dragon.

Bonnie Harris manages to separate from The Fiend as a result, and The Devil appears to take her soul. The Devil is confronted and defeated by God, who sends Harris to eternal damnation and Dragon to Earth.

The Dragon returns from Hell with withered arms. Chris Robinson tells his story to Alex Wilde, including his role as Star. After a date with Dart, Justice is attacked by a jealous Kill Cat. Dart, as a result, breaks several bones in Kill Cat's body. Rapture's unborn child begins to kick, causing her internal damage.

Phil literally explodes all over Niseman and Dragon arrives at the hospital a few minutes too late to witness the birth of his son. Kill-Cat is hospitalized, while Dragon learns the truth about Rapture and Peter Klaptin (who is meanwhile being held captive by a shadowy figure). Dragon then prepares to propose to Rapture, only to discover that his son is dead. In reality, the newborn has been captured by The Covenant of the Sword and was replaced with an artificial "dead" fetus.

Brainiape, who is revealed to be Adolph Hiter's brain preserved within the body of a gorilla, is sent on a mission to kill the Dragon by Cyberface, the defacto leader of the Vicious Circle. Cyberface is decomposing, so to save him, Abner Cadaver summons the creatures of the undead, who attack the world-renowned paranormal investigator Hellboy.

Brainiape causes Hellboy to do battle with The Dragon, and then summons a giant sea monster, which eats them both. Unaffected by the Vampire's bite, Cyberface is only restored to health after Abner Cadaver imbues him with the life force of those around him. Hellboy's government allies kill the sea monster, and the emerging Dragon impales Brainiape's brain with a knife, freeing Hellboy from his mental control.
