Spawn cannot find peace. First he discovers the remains of his human body; then Malebolgia torments him. He's transported to Simmonsville, a town built from Psychoplasm and Al's memories. When the Redeemer attacks, Spawn flees to his alley. Sustaining grievous injuries, Spawn is knocked unconscious as the Redeemer prepares to deliver the killing blow.

The homeless distract the Redeemer while Spawn recovers. As he fights, Spawn gradually realizes that his foes powers are being depleted. With a final blast, he downs the Redeemer, who is transported back to his heavenly creators. Afterward, Spawn returns to Simmonsville and destroys this reminder of a happier time. He accepts that sorrow and loneliness have become his fate.

He teaches Spawn how to use his uniform more effectively, which comes in handy immediately when they are attacked by a street gang. Terry is in a world of trouble -- framed by the CIA and forced into a midnight rendezvous with the Mob in order to get his kidnapped wife and child back. Spawn decides to help, but is stopped by Twitch.

Terry, mistakenly targeted as Spawn, stands no chance against Overtkill. Luckily, Spawn escapes from Twitch in time to help his old friend. He removes Terry from harms way, while his cape intercepts one of Overtkill's laser beams. Twitch, in hot pursuit, arrives on Spawn's trail just in time to fire a well placed shot in Overtkill's ear, disrupting his programming.

The homeless prevent Sam and Twitch from arresting Terry and Spawn. Wanda and Cyan had not actually been kidnapped, but Terry is still wanted by the Mob, CIA and police. Spawn threatens each of Terry's antagonists with the release of ruinous documents if they continue to pursue him. Thanks to Spawn's bluff, Terry is cleared.

Tremor, a new force on the street, arrives in Spawn's alley, looking to expand his turf. Spawn launches a preemptive attack, luring in Tremor with a decoy, then choking him with his chains. Tremor than reveals that his real foe is Twistelli. It turns out that Tremor had been a normal man who Twistelli transformed into a hulking brute, and now he's hell-bent for revenge.

Deep in Hell, Malebolgia gleefully details how Al is being groomed to lead Hell's armies. Meanwhile, in Heaven, Spawn is being interrogated about his clash with Angela. Back in the alley, Spawn spurns a plea for help from a down-and-out man. The man later commits suicide, leaving Spawn to ponder his earlier refusal.

Spawn resurrects a bum killed in the local gang wars, weakening himself with the power drain. At that moment, he's attacked by The Curse, a religious fanatic convinced that he'll gain entrance into Heaven by killing demons. In a small Alabama town, a well-respected police officer abuses his two young boys. The youngest, Andy, gives shelter to the battered Spawn. Spawn takes action, brandishing the father with the words "I beat my kids." This only increases the violence, which pushes the older boy to grab his father's pistol and end the abuse forever.

Spawn's long trek back to New York leads him deep into Klan country, where a local chapter is planning to kill the county's last remaining black family. Spawn traps the leader in the family's house and transforms him into a black man. When the rest of the Klan arrives, the costumed redneck finds out how it feels to be on the other end of the lynching rope.

Spawn returns to New York and reclaims his throne. Meanwhile, Heaven has selected another Redeemer. This one is pure of heart, and therefore more powerful. Spawn is easily defeated, and his ravaged armor momentarily shuts down. The Redeemer then teleports himself away with Bobby, the bum that Spawn brought back to life.

After his costume recovers, Spawn invades Heaven's terran headquarters. He demands Bobby's release, threatening an oddly powerful old lady with death if he's not heeded. Luckily, the threat works. The Redeemer pursues them, but this time, Spawn transports Bobby and himself away from the fight and into the alleyways.

Spawn ponders leaving town because his presence endangers the homeless. A spy tells his shadowy boss of Spawn's plans. The lackey sets up a meeting in the sewers with Spawn and his boss, who turns out to be the Violator. As they battle, the Violator warns Spawn that if he leaves town, his homeless friends will die.

As Spawn and the Violator battle in the sewers, a water main ruptures and washes them away. Violator, worried by Spawn's rapidly developing armor, enlists Wynn to help him destroy Spawn. He advises Wynn to befriend Terry while leaning on his family, hoping to enrage Spawn and make him vulnerable. Cog urges Spawn to think before he acts, lest he be goaded into a foolhardy attack.

At the police department, Burke threatens to expose Chief Banks with the file Spawn gave him, which links Banks to the child killer Kincaid. Banks faces Spawn, but loses his nerve and reveals that he works for Wynn. Enraged, Spawn confronts Wynn but Terry bursts into the office and demands Wynn's release. Spawn believes Terry has befriended Wynn.

Spawn is pained to see his old friend Terry protecting Wynn. Wynn's troops fire on Spawn, forcing him to flee with his prey. Pursued by aerial airships, Spawn finally leaves Wynn behind to escape. Now suspicious of Terry, he warns Wanda about him. The visit leaves Spawn shaken by the abject terror he engenders in Wanda.

The windfall allows the family to forget about day to day worries and appreciate their many blessings.

As Cy-Gor rampages, the Curse prepares to get to the root of Spawn's evil by studying his body. Left alone, Spawn severs his own hand and uses it to crack the tank holding his uniform.
