Tiffany, a firecracker with a taste for blood, is determined to get the job; and her path to success leads straight through Spawn. Coming upon him in his weakened state, Tiffany confronts Spawn and a fierce battle ensues with Tiffany about to decapitate Spawn. Meanwhile, Sam and Twitch wonder why Banks took the fall when others were implicated in their investigation.

Tiffany lops off Spawn's head, only to see it turn into a mass of maggots in her hand -- she's been tricked! Spawn attacks, while the evil creatures in the woods gather to aid him. Together, they reduce the angel to bloody scraps. Spawn returns to New York and meets Cog, who warns Spawn that he must gain control of his costume. Meanwhile, Sam shares with Twitch his dream of opening his own detective agency.

Tremor, thirsting for revenge against the man who ruined his life and killed his family, picks off Twistelli's hit men. Twistelli saves his own life by telling Tremor that his brother still lives. Tremor seeks Spawn's help. Spawn at first refuses, but changes his mind when Tremor brings back one of Twistelli's hit men who's willing to talk. Meanwhile, Terry's illness is getting worse.

In his mansion, Twistelli waits for Tremor to show up,planning to capture him and send him to Sicily to replace the missing Overtkill. When Tremor arrives, he finds his brother acting as Twistelli's bodyguard. Spawn appears and warns Twistelli to stay away from Wanda. Meanwhile, Clown enlists the help of Wynn to bring down Spawn. And Cog wonders if Spawn's costume has overpowered its master.

In his office, Wynn plots against Sam and Twitch, setting up a secret meeting where they are to be eliminated. Meanwhile, Terry has finally linked Wynn to illegal activities, but blacks out due to a mysterious illness. In Rat City, Spawn builds a hideout. Cog wonders who is building it - Spawn or his costume.

Spawn and Terry both battle their personal demons. Spawn's suit has completely taken control of him. He hemorrhages and sprouts growth until he is helped by Cog. Terry blacks out and is struck by a semi-. The accident lands him in the hospital. Cy-Gor is in New York city looking for Spawn.

Terry learns that he will die in two months from a malignant brain tumor. Spawn is torn apart by the belief that Terry's a traitor, but he uses all his power to heal him. This sends Spawn hurtling to Hell's second level, where he is immediately attacked by creatures trying to drain his life-sustaining necroplasm. Then suddenly, memories of past injustices provoke Al into a rage and he rises to fight again.

Spawn freefalls through the levels of Hell where he he undergoes a physical and psychological baptism. Layers of Al Simmons' life are peeled away and his weaknesses are exposed. Finally, Malebolgia rips Al's heart out. Meanwhile, Terry's doctors are still puzzled by his miraculous recovery while Cyan refuses to give up a filthy shoelace she found at the hospital. Elsewhere, Sam shows Twitch their new '55 Chevy car.

Cyan's shoelace is the only physical clue to Terry's mysterious recovery from cancer. Terry's nightmares mirror Al's experiences. Spawn enters Hell's fifth level where he is revered as a returning savior. He is forced into a confrontation with the Savage Dragon and defeats the dragon for the position of the Messiah. The dwellers of Hell turn on both of them and stone them to death - or do they?

Al Simmons is returned to his demonic birthplace where he is tormented by a demon in the likeness of Wanda. When he realizes that Malebolgia has tricked him, he destroys the demon and passes another of the Malebolgia's tests. Malebolgia then agrees to leave Wanda alone in exchange for Al's loyalty and servitude. Spawn then enters Terry's dreams where he suddenly awakens knowing that Al is "alive."

To confirm his dreams of Al, Terry searches the alleys for his dead friend. When Cog intercepts and questions Terry about his motives, he ends up mediating their encounter and saving Terry from Al's jealous rage. After Terry shows Al proof of the case he has against Wynn, Spawn helps Terry on a dangerous mission. Meanwhile, Sam & Twitch wait for the "informant" regarding information regarding information about the Chief Banks - Senator Jennings conspiracy.

When Terry meets with Spawn to plan their mission, he tries to understand his friend's chosen hideout as well as their new-found friendship. When he stumbles onto the worms, Al's reaction confuses even more. Terry and Al carry out their mission to document Wynn's involvement in the arms trade. Upon seeing the evidence, Al sends an explosive message to implicate and endanger Wynn's global network infrastructure.

Major Forsberg, a military strategist, has been kidnapped and imprisoned by Jason Wynn, a key figure in U.S. foreign intelligence. Forsberg had determined the insights of the great conqueror Genghis Kahn and relayed some of this to Wynn. When Forsberg denied having full knowledge, his family is murdered by Wynn. Forsberg believes that Wynn is inhabited by Kahn's spirit. Spawn frees Forsberg from Wynn's prison. Meanwhile, Clown continues to direct Wynn to destroy Spawn and Cy-Gor awaits Spawn's return to Rat City.

Although extremely traumatized, Forsberg, the military strategist, is successfully transported to Spawn's alley for safekeeping until Terry and Al figure out how to use him to their advantage against Wynn. When Cy-Gor makes a surprise attack on Spawn, Forsberg huddles in the background watching the battle. He then returns to reality with some startling information about Cy-Gor's origin and connection to Wynn.

Eddie and Andy, the abused children who killed their father, escape their detention and find their way to New York City in search of their hero, Spawn. There, they are befriended by Snake who offers to employ Eddie to courier illegal packages for him. Snake forces Eddie to lead him to Spawn where a confrontation ensues. Snake is killed and Andy is wounded. After medical treatment, the boys return to the detention center, forever stripped of their innocence.

Spawn pressures Terry into making a final move against Jason Wynn using Major Forsberg's information. Later that day, Cyan appears to be kidnapped by Spawn. Confused and distraught, Terry and Wanda try to understand their relationship with Spawn and explain it to police as they search the alleys for Cyan. Wynn finds out that Terry is leaking information to Wynn's "clients." And a scared and hungry Cyan is forced to color pictures for the real kidnapper - the Clown.

While the police search Spawn's alleys looking for Cyan's kidnapper, Wynn directs a surveillance of his own on Terry with orders to find out why the police are looking for him. Spawn then finds Cyan and the Clown who taunts him into a battle. After Spawn uses up a portion of his energy to destroy the Clown, he brings Cyan to her parents.

After Cyan is returned safely to her parents, Al has a long talk with Granny Blake and as usual, her words of wisdom calm him while creating inner confusion. When he gets back to the alley, the alley bums have rebuilt his throne which seems to hypnotize him as he settles into it. The Clown appears in Spawn's trance and reveals startling facts about his past and future. Meanwhile, Wynn confrims Cy-Gor's remains are safe at a testing facility and orders the disappearance of Terry.

Spawn confronts Jason Wynn only to have his vengeful anger stymied once again. Wynn informs him that if he dies, Terry, Wanda & Cyan will be eliminated. Knowing that he doesn't bluff, Spawn turns it into a catch-22 situation. Later in the alley, chaos erupts when Spawn and Angela are thrown together to discuss the unraveling of the universe. An unknown force then heals Al's face. Meanwhile, Terry is informed about a neighborhood petition to force them out of the area.

A shaken Jason Wynn tells his security forces about a "terrorist threat" on his life and they reinforce the area. Meanwhile, Spawn's face has healed itself. Although startled and confused by this new turn of events, Al forms plans to further intimidate Wynn to ensure Wanda's safety and to win her back. Later, Cog counsels Spawn to get him to reconsider his decisions. Elsewhere in the city, Sam & Twitch are pondering how some of their previous cases seem to be tied together when they receive a hot tip and head to the alley.

By morphing his symbiotic suit to match the soldiers, Spawn infiltrates the compound where Wynn has reinforced security. He successfully severs communication and power systems to cripple the battalions, leaving a terrified Wynn trapped. Spawn threatens Wynn with the promise to undermine his operations until nothing remains of his international empire. In the alley, Sam & Twitch get no information from the bums about Spawn. Cog then fills them in on facts that create more questions.

When Cog promises to answer Sam and Twitch's questions, they follow him into the deepest parts of Rat City. Twitch takes notes as Cog tells the tale of Spawn's confused existence as it relates to them. They view Spawn's throne and hear of his life as it was on earth before he returned as a Hellspawn. At the tour's end, Cog tells the detectives that Spawn has chosen them to help recapture his lost humanity.

Cog agrees to accompany Sam and Twitch back to their office to answer additional questions but vanishes from the moving '55 chevy. Back in Rat City, Al Simmons prepares for his reunion with Wanda like an eager groom. When he discovers that his face has returned to its burned appearance, he vents rage and frustration. Terry is uncooperative when Al wants to continue their plan to bring Wynn down. Meanwhile, Wanda becomes concerned when Cyan flies into a rage when the dog snatches her shoelace & pacifier.

The bums gather to commiserate on their lives and share stories and philosophies about their religious beliefs. Spawn diplomatically joins into the conversation and avoids heating the age old debate of God versus the Devil. Later, in another part of the alley, a gang of bums plot to rob and kill one of their own. When Spawn steps in to question their motives, they open fire on him. He collapses, wondering why his costume isn't defending him against this assault.
